Some of the most exciting things we do take place every day in our Rehabilitation Therapy Center. Our in-house therapists and nursing staff work as partners to treat patients who may have lost strength and mobility due to illness, trauma, or surgery. It is easy to imagine the delight of both the therapist and patient when that patient—who may have been incapacitated in one way or another—becomes more independent through concentrated effort and care.
Rest and relaxation are crucial to a happy, healthy life. At Plum Tree, we offer a variety of resident-centered activities for residents to choose from. Whether you prefer individual activities in the comfort of your own room or choose to partake in our daily social group activities, our staff is here to help promote your well-being.
My sister-in-law has had a very rough time. As she was recovering from cardiac arrest in August 2020, she had a stroke in November. The stroke left her paralyzed on the right side and with expressive aphasia. A few weeks after starting her recovery at San Jose’s Good Samaritan Hospital, she was moved to Plum Tree. Her siblings and I could not be happier about the active care she has received. Of course, COVID has created many difficulties. We’ve not been able to visit; Zoom and Facetime don’t make up for touching and hugging. The Plum Tree staff have been called on to be our eyes and ears, to explain her progress sometimes in time-consuming calls. The staff has been everything we’d desire: informative, professional, kind, and caring. Mark, who heads up her therapy, and Everett, who has planned her discharge, especially, have kept us informed of her progress and carefully prepared us for her release. Christina from the business office has provided needed administrative backup. It’s been reassuring to know that even in this COVID-time, my sister-in-law has received excellent therapy and care.

Denisse Romero1 StarI been trying to call plum tree and no one answers. I wanted to know the status of my mother in law. I left multiple messages and no one has reach me. She supposed to get discharged like 3 weeks ago and now there saying that we have to have all the medical essentials at home of what she’s going to need . Supposedly they called her insurance to order what she needs and it’s been 3 weeks or more that they order it and no one has contact us. My mother in law it’s going through depression now and every time I go she looks sad and not we’ll groom.Tuesday 16th April 2024

We had our dad there about 18 days and from the second days his personal items started disappearing, when I talk to the social worker the first thing I heard was if it's not on the belongings list than we have no responsibility!!
Care quality was poor, if your patient is not able to walk or talk could left alone and hungry. They never showered him during the whole time was there. One of the nurses was angry why my dad ask her to put his phone in charge and throw his charger on the floor. When we asked why she did that, they denied it.
I even spoke with the family of the patient that was next to my dad and they were saying the same thing.
When we asked to discharge our dad from there, social worker first said he must stay otherwise we will not give him any services, and when I asked her to give me the print of the report of what was done for my dad, she said I don't have time for today I will let you know when I get it done. They also charged my mom $13 for the print papers.Sunday 4th February 2024
matt kells1 StarThe ineptitude is unfathomable. Takes over an hour to respond when patients are in need. Most nurse's are unlicensed and still students. They loose medications and say it's missing. The pharmacy takes ocer8 hours. The pharmacy also makes recommendations outside of doctors orders. The Management does little to control staff. There is zero communication between staff and managers. As a current patient I do not feel safe in any way. Go anywhere but here. Come here to die.....zero confidence in the care level available. Stay awaySaturday 16th March 2024
Debby Woodward5 StarPlum Tree Care Center took care of my sister while she was with hospice. I live out of state and was able to be with her for three days while she was in their care. Although she did not pass before I left, I was at ease leaving her in their care because of what I saw while during my time with her. Both day and night shift nurses, aids and other staff were so attentive to her and the other patients. They were always busy making sure their patients had their meds, keeping them clean and showing compassion to them and their families. The facility was exceptionally clean and they all took pride in their jobs. I would highly recommend Plum Tree and their staff to anyone needing their services.Thursday 16th November 2023
carla Roberson1 StarI would not place a loved one here.My mom was there for a month not enough staff, they don't answer the phone or return calls, somebody broke her glasses and tried to hide them ,they don't check on patients in their rooms to often ,lack of training of the staff you ask a question and just get a blank look on their faces they don't pay attention to what the family tells them what they see good at finger pointing . Dirty cups ect.may sit in their rooms for days .
